The Annual General Meeting of the Canadian Association of Specialized Kinesiology, will take

place on Monday, September 22nd in Surrey, BC.

In the event you are unable to attend, you may send a proxy with another person who will be

attending, so that your vote may be counted. Simply fill out the form below, and send it with the

person who will be attending. The person carrying the proxy is responsible to register it before the

start of the meeting. Anyone attending and voting at the AGM is required to be a member in good

standing, therefore please ensure your dues are paid or included with the proxy in order for your

vote to count. Emails are not acceptable, as we are required to have your signature.
